Notes: We accept and are providers for several different vision insurance plans. Our office is required in some cases to have prior authorization before providing your services. When scheduling your appointment with our office, please know the name of your vision insurance. Sometimes the name of your vision insurance carrier is separate from your medical insurance. If you are unsure of the carriers name or type of vision coverage, your human resources office or payroll clerk can assist you with this information.
